This is a sample communication we recently sent to a student after their course completion was rejected in Florida.
Thank
you for your quick response. We see that your last name matches your
identification, however the state is still not accepting your last name
as “-------- -----------”. This may be caused by one of the following issues:
1. You have or had a FL insurance license under a different last name
2. Your MyProfile account at https://dice.fldfs.com/public/pb_index.aspx doesn’t have your correct last name
3. You had your fingerprinting done and it wasn’t reported under your full last name
You will need to contact FLDFS at AgentLicensing@myfloridacfo.com and
advise them that your Educational Provider is unable to report your
course completion due to an error, "last name doesn’t match" the SSN.
Once this issue of your last name has been resolved we can report you
to the State of Florida.
